1997 f-350 engine failure

I have a 97 7.3 that smoked black then stalled out. They are from out of town and the trucks passenger side bank has both melted pistons and cracked pistons. I have not inspected the drivers side but compression was good around 380 psi. I can't get the full story but this is a reman engine that has blown up a couple of times before. They tell me that they have had the passenger side head replaced already. I have inspected the injectors and did not find any cracked tips, I did find one injector had the upper o-ring with some damage. I have checked the fuel and it looks clear and free of water. What would cause just one bank to go down like this? Could the truck have had a leaking injector raising the oil level causing the engine to run on unmetered fuel ? Have you seen this before on the 7.3? any advice?
